- Set autopilot to 2000 feet to reduce work load.
- Autopilot was somehow switched off, leading to a gradual descent.
- Crew did not notice, ATC did but was not responsible for notifying plane of altitude. The controller did contact but was told all OK.
- 30 seconds later place crashed killing 99 out of 176.
